Important Safety Information about the OneTouch® Ultra® Blood Glucose Meter

If you use a OneTouch® Ultra® Meter, we want to inform you that—on rare occasions—users of the OneTouch Ultra Meter have experienced a change in the unit of measurement of blood glucose readings from mmol/L to mg/dL. The unit of measurement change can occur when the meter is dropped while in use or when the battery is changed with the meter on.

While it is rare, this kind of change in unit of measurement from mmol/L to mg/dL could lead the user to think that their blood glucose result is high. This is because test results expressed in mg/dL do not contain a decimal point and are 18 times higher than the same results expressed in mmol/L. For example, a result of "6.0 mmol/L" would display as "108 mg/dL," which might be misinterpreted as "10.8 mmol/L." This could potentially result in a user altering their treatment regimen, such as injecting more insulin than is needed.

LifeScan Will Replace OneTouch Ultra Meters in the UK and Ireland
LifeScan will conduct a programme in the UK to replace OneTouch Ultra Meters (as well as the no longer manufactured InDuo and PocketScan Meters) with a OneTouch Ultra2 Meter free of charge. The company will be communicating with patients, healthcare professionals and pharmacies about the replacement programme, which will begin in early 2007.

What You Should Do
LifeScan has initiated a replacement programme for all patients using OneTouch® Ultra®, InDuo® and PocketScan® meters with a new OneTouch® Ultra®2 Meter where the unit of measure cannot be changed. To replace your meter:

  • Telephone free on 0800 121200 (UK) or 1 800 535 676 (Ireland).
    Telephone lines open: Mon-Fri 8.30am - 6.00pm, Sat 9.00am - 1.00pm

While you wait for a replacement meter, you can continue to use your current meter but should check that it is reading in mmol/L units of measurement each time you test your blood glucose, particularly if you feel that you are having a hypoglycaemic reaction.

If your meter displays mg/dL, contact the LifeScan Customer Care line immediately on 0800 121200 (UK) or 1 800 535 676 (Ireland) for assistance.

mmol/LHow to Check Your Meter's Unit of Measure
Each time you turn on your meter, the unit of measure will appear on the display with the symbolsymbol. The unit of measure will also appear under every test result.

If the meter is set to the right unit of measurement, "mmol/L" will appear on the display. The test result will include a decimal point (Fig. 1). In this case, continue to use the meter while you wait for a replacement.

mg/dLIf the meter is set to the other unit of measure, "mg/dL" will appear on the display. The test result will not include a decimal point (Fig. 2). In this case, do not use the meter and call LifeScan customer care line 0800 121200 (UK) or 1 800 535 676 (Ireland) immediately for a replacement meter.

No other LifeScan Meter are affected by this issue, including: OneTouch® UltraEasy™, OneTouch Ultra2, and OneTouch® UltraSmart®.
